Invisalign in Japan: what Tokyo and Osaka clinics charge
University dental hospitals' and English-speaking clinics' own fees for Invisalign, aligners and braces, every aligner brand that treats in Japan with its yen price, what health insurance covers, and what the Japanese Orthodontic Society says about aligners sold without a dentist.
Clinics publish Invisalign from about 440,000 yen for mild cases to ¥1,199,000 for a full case; university dental hospitals list aligners from ¥282,370, with examination and treatment fees on top. The aligner brands you choose directly start at ¥300,000 (excl. tax) (hanaravi), 300,000円(税込) (Zenyum), ¥308,000 (DPEARL) and ¥330,000 (Oh my teeth), each as it states it (with or without tax). Health insurance does not pay for orthodontics for appearance, and treatment runs on regular check-ups.

What health insurance covers
- Invisalign Japan: “Orthodontic treatment is basically self-pay treatment, to which insurance does not apply.” (our translation of “歯列矯正治療は基本的に保険が適用されない「自費診療」になります。”, Invisalign Japan)
- The Japanese Orthodontic Society: “Orthodontic treatment is generally outside insurance, and is covered by insurance only in the following cases.” (our translation of “矯正歯科治療は一般的には保険適用外ですが、下記の場合に限り保険診療の対象となります。”, Japanese Orthodontic Society) Among them: “(1) orthodontic treatment for a bite abnormality caused by a disease the Minister of Health, Labour and Welfare designates” (our translation of “① 「別に厚生労働大臣が定める疾患」に起因した咬合異常に対する矯正歯科治療”, Japanese Orthodontic Society)
- The Japan Dental Association: “Orthodontic treatment is generally not covered by insurance and is paid by the patient. Its cost differs by clinic, method and length of treatment, and also by region.” (our translation of “矯正歯科治療は、一般的に保険はきかず自費の支払いです。その費用は、医療機関、治療方法、期間により異なり、また地域によっても異なります。”, Japan Dental Association)
What the Japanese Orthodontic Society says, and what an aligner is under Japanese law
In each body's own words, with our translation:
- The Japanese Orthodontic Society, 2022. “Aligner treatment in which aligners are shipped straight to patients through a large courier, neglecting proper examination, tests, analysis, diagnosis and follow-up by an orthodontist (a dentist who has completed specialist orthodontic training), can cause unexpected major problems and is very dangerous from a dental point of view.” , Japanese Orthodontic Society) “Tooth movement with aligners made on equipment not recognised as a medical device (classed as general goods) is not guaranteed, and the dentist who moved the teeth bears full responsibility for bite problems afterwards.” (our translation of “医療機器として認められていない機器で作製されたアライナー(雑品)による歯の移動結果は保証されず、アライナー(雑品)使用後の噛み合わせの不具合等は、歯の移動を行った歯科医師が全責任を負うことになります。”, Japanese Orthodontic Society)
- Device status. The society's 2014 notice: “Custom-made orthodontic appliances, whether made in Japan or abroad, are not medical devices under Japan's Pharmaceutical Affairs Act.” (our translation of “カスタムメイドの矯正装置は国内外で製作されたものを問わず日本国の薬事法上の医療機器に該当しません。”, Japanese Orthodontic Society) Invisalign Japan, in 2025: “We have decided to state again on our website that all our orthodontic appliances are made with materials and software approved or certified as medical devices.” (our translation of “当社の矯正治療装置は、全て医療機器として承認、認証を受けた材料及びソフトウェアで作製されていることを当社ホームページにて改めて公表することにいたしました。”, Invisalign Japan) The advertising guideline asks clinics to “State clearly that the unapproved product used has not been approved under the Pharmaceuticals and Medical Devices Act.” (our translation of “用いる未承認医薬品等が、医薬品医療機器等法上の承認等を得ていないものであることを明示すること。”, MHLW) so a clinic page reads: “Invisalign is a custom-made medical device that has not been approved under Japan's Pharmaceutical and Medical Devices Act.” (Rodin Dental)
Every brand's FDA reports and recalls are on the safety record.
Starting treatment in Japan if you may leave: the check-ups and the refund rules
Aligners are not a single appointment. Invisalign: “Your doctor will also schedule a series of regular, face-to-face check-ups to give you each new batch of aligners and monitor the progress of your new smile.” (Invisalign)
- Refunds. Kanagawa Dental University's Yokohama clinic: “Treatment fees once paid are not refunded for any reason, so if you may find it hard to keep attending because of a move or the like, please talk to your doctor beforehand.” (our translation of “なおいったんお支払い頂いた治療費は、理由の如何を問わず返金いたしませんので、 転居などで通院が困難になる可能性がおありのときは、あらかじめ担当医等にご相談くださいますようお願い申し上げます。”, Kanagawa Dental University)
- Finding care as a visitor. “For emergency support (illness, disasters, etc.), please call the JNTO Japan Visitor Hotline 050-3816-2787. 24 hours, every day.” (JNTO) Its medical search lists “Base medical institutions, selected by prefectures, that can provide services to foreign patients (including clinics and dental clinics).” (JNTO)
- The CDC. “Travelers may need to get health care in the United States if they develop complications after returning. Follow-up care for complications might be expensive.” (CDC)

In its 2022 position statement: “Cases have appeared of mouthpiece-type products sold online with no dentist involved, claiming to improve the alignment of the teeth.” (our translation of “インターネット上で、歯科医師が介在しない形でマウスピース型製品が販売され、歯列の改善への有効性を謳うケースが出てきています。”, Japanese Orthodontic Society) And: “Aligner treatment in which aligners are shipped straight to patients through a large courier, neglecting proper examination, tests, analysis, diagnosis and follow-up by an orthodontist (a dentist who has completed specialist orthodontic training), can cause unexpected major problems and is very dangerous from a dental point of view.” , Japanese Orthodontic Society)
What if I start treatment in Japan and then move away?
